Apply For a U.S. Passport

Exeter Public Library (Rhode Island) is proud to be a Certified Passport Acceptance Facility. We’re an appointment-only facility. Please review the information on this page fully, then click here to make an appointment online. Appointments are available 14 days out as follows:

  • Tuesdays 12-6:30 pm (last appointment at 6 pm)
  • Wednesdays 10 am-4:30 pm (last appointment at 4 pm)

Please note: We do not offer passport photo service on site. Staples, Walmart, and AAA are some businesses that provide this service.

 

Avoid Passport Scams!

Travel.state.gov is the only website for official passport information. Avoid all other websites, and do not pay fees to any other companies or entities (even if they're local or well-known). Keep in mind that appointments and forms are free, all regular fees must still be paid in-person at the acceptance facility, and it is already part of our service to review your application for full compliance. Passport Renewals

Eligible passport renewals must be completed by the passport holder. Acceptance facilities no longer handle eligible passport renewals. Eligible renewals include 10-year adult passports that were issued less than 15 years ago. Click here to view instructions for renewing your passport.

 

All Applicants Need to Bring...

  • Form DS-11 filled out, legible and complete. Use the online form filler (print out single-sided) or fill out the paper form using black ink only. Paper forms are available at the library and downloadable forms can be found on the Travel Website. Remember to also fill out a lost or stolen passport form (Form DS-64) if your previous passport is lost or stolen and you have not reported it. Please note that your appointment does not allow time to fill out forms and may be forfeited if there are applicants waiting and there is not enough time.
  • Passport photo that meets requirements. Unacceptable photos are the most common reason for passport application delays. Please review passport photo requirements to ensure that your photo is acceptable according to the guidelines.
  • Original proof of U.S. citizenship. An original document proving your citizenship, such as birth certificate, naturalization certificate, or previous U.S. passport, is required and will be submitted with your application. Photocopies are not acceptable.
  • Photo ID. A driver’s license is adequate. Click here to view the complete list of acceptable IDs. For minors, both parents’ IDs are required.
  • A check or money order. The State Department only takes checks or money orders for passport applications.

 

Applicants Ages 15 and Under Will Also Need...

  • Original proof of parental relationship. A birth certificate in most cases. Please note that a birth certificate can serve as proof of both citizenship and parental relationship.
  • Child and both parents/guardians present. Or…
  • If only one parent/guardian is present, Statement of Consent (Form DS-3053) from the non-present parent/guardian, filled out and notarized, as well as a photocopy of the non-present parent/guardian’s ID that is used for notarization (front and back on one sheet of paper).

 

Applicants Ages 16 and 17 Should...

  • Bring their own photo ID if they have one.
  • Be accompanied by at least one parent/guardian with a photo ID.
